Microchip Registration/Transfer Instructions

DOCUMENTING MICROCHIPS IN ASM

 

1. Log into ASM and click on the foster dog's record.

 

2.  Click on the drop down menu labeled "Health and Identification.

 

3.  Click on the check-box to the left of "Microchip", insert the date, then the chip number in the appropriate field.  The name of the chip manufacturer will automatically populate to the right of that field.

 

4. Click "Save".

In the example above, you'll notice that the chip manufacturer has been identified as HomeAgain.  It doesn't matter who the chip manufacturer is. ALL CHIPS (including HomeAgain chips) need to be registered via HomeAgain's website. HomeAgain is a national chip database service.

Microchips need to be registered two times with HomeAgain:

• The first time is when a microchip has been assigned to and implanted into an MLRR foster dog. During this HomeAgain registration, MLRR is designated as the owner of the dog and the name of the dog is assigned to this chip. (See instructions below.)

• The second time is when the dog is officially adopted. During this HomeAgain registration, the ADOPTER is designated as the owner of the dog and MLRR is recorded as the secondary contact in case the dog is lost and the owner cannot be reached. (See instructions below.)

Each time the chip is updated on HomeAgain's website, the action needs to be recorded in ASM.

1. When MLRR is recorded as the current owner on HomeAgain's website, click on the drop-down menu located to the right of "Microchip Location" and select MLRR. Enter the date into the field labeled - Microchip to MLRR Date. 

 

 

3. When the Adopter is registered as the current owner on HomeAgain's website, click on the drop-down menu located to the right of "Microchip Location" select New Owner. Enter the date into the field labeled - Microchip to Owner Date.

 

Following these steps will keep track of each dog's microchip registration status.

HOMEAGAIN MICROCHIP REGISTRATION

 1. Click Login on the HomeAgain website.

 

2. Enter the user name and password for MLRR's account and click the Login button.

 

3. On the left-hand menu, select - Manage HomeAgain Business.

 

4. On the left-hand menu, select - Enroll Patients Online.

 

5. If MLRR is the current owner of the dog (not officially adopted), enter the owner information like shown below. Make sure your web browser doesn't auto-populate information into these fields and double check for accuracy.

Enter the dog's name in the Pet Name field.
Select "dog" from the drop down menu under Species.
Paste the chip number from ASM into the Microchip # field.
Double check for accuracy, then click the Checkout button.

 

 6.  If the chip is a pre-paid MLRR chip, you will see the screen below, indicating the grand total is $0.00.  

Click Checkout.

 

7.  When the registration is complete, you will receive the following confirmation screen.

 

8. If the ADOPTER is the current owner of the dog (officially adopted), enter the owner information like shown below. MLRR is recorded as the alternate contact in case the dog is lost and the owner cannot be reached. Make sure your web browser doesn't auto-populate information into these fields and double check for accuracy.

Enter the dog's name in the Pet Name field.
Select "dog" from the drop down menu under Species.
Paste the chip number from ASM into the Microchip # field.
Double check for accuracy, then click the Checkout button.

 

9. If MLRR was previously registered as the owner of this dog, you will receive the following pop-up notification.

Select Yes and click Continue.

 

10. If the chip is a pre-paid MLRR chip, you will see the screen below, indicating the grand total is $0.00.  

Click Checkout.

 

11. When the registration is complete, you will receive the following confirmation.

HomeAgain will send a welcome email to the new owner, inviting them to establish a HomeAgain account and participate in additional services.

 

12. If the chip is NOT a pre-paid MLRR chip, you will see a charge of $9.95 during checkout.

Complete the chip registration by paying for the chip, using your personal credit card. Take a screen shot of the confirmation notification. Submit a Reimbursement Request Form and attach that confirmation notification as receipt of payment.
